Care professionals: your document in 30 seconds, in the language of your visit

Visit note, Handover note, and Care plan: dictate or paste what was said, and Notae structures the document in your own vocabulary. You review it, validate it, and send it.

Between rounds and handovers to the next team, noting what was done and observed for each patient takes time the pace of care doesn't always allow. Notae turns what was said or observed during the visit into a structured document, in the formats a care team actually uses.

Three formats, your vocabulary

A nurse Notae account only sees these three formats — never the formats or vocabulary reserved for another profession, and vice versa.

  • Visit note — Vitals/observations (if taken), care provided (dressings, medication if mentioned), general condition reported (by the person or observed), and handover to the team (points to watch).
  • Handover note — What was done (factual summary), what remains to be done, points to watch (without a diagnosis), and instructions for the next team.
  • Care plan — Needs identified (reported or observed), care goals, planned actions (frequency, means), and scheduled reassessment.

Before / after, on an example

What you dictate at the end of the exchange:

"how are you feeling this morning? a bit tired but the pain in my arm has eased — let's check blood pressure and temperature, 120/80 and 36.8°C, stable, I'll redo the dressing now — I'll note everything for the afternoon team and come back before noon"

What Notae produces (Visit note format):

Vitals : blood pressure 120/80, temperature 36.8°C.

Care provided : dressing changed on the arm.

General condition reported : fatigue noted, arm pain decreasing.

Handover to team : vitals stable, dressing changed, return before noon for treatment.

To be validated by the care professional.

Frequently asked questions

Does Notae replace the care professional's clinical assessment?

No. Notae turns what was observed or said during the visit into a structured document, without ever inventing a vital sign, a task performed, or a medical diagnosis that wasn't mentioned.

Does a nurse account see formats meant for other professions?

No. Every Notae account has a profession chosen at signup. A nurse account only sees the formats and vocabulary specific to their profession; it has no access to any format or text reserved for another profession (for example a notary), and vice versa.

Do handovers to the team stay confidential?

Yes. Notes and handovers stay in your secure account; you choose what is shared with the team, on which medium, and with whom.
