- Speech-language assessment — Reason for consultation (reported concern, referral if provided), history (language development, relevant background — only if mentioned), functional observations (articulation, voice, fluency, spoken/written language, comprehension, swallowing — as reported), rehabilitation goals (functional, measurable where possible), and follow-up plan (frequency, methods considered).
- Session note — Session goal, exercises practiced (type, materials), observations (progress, difficulties, reactions), home practice, and next session.
- Report (school, parents) — Context of the follow-up (frequency, duration), progress observed (in plain language, no jargon or diagnosis), practical recommendations (for the classroom or home), and next steps considered.

Frequently asked questions
Does Notae replace the speech-language therapist's expertise?
No. Notae turns what is said or practiced during the session into a structured document. It never states a diagnosis (dyslexia, dysphasia, spectrum disorder…) that hasn't already been made by someone else, and reminds that the document must be validated before it is sent.
Does a speech-language therapist account see formats meant for other professions?
No. Every Notae account has a profession chosen at signup. A speech-language therapist account only sees the formats and vocabulary specific to their profession; it has no access to any format or text reserved for another profession (for example a lawyer), and vice versa.
What does Notae share with the school or parents?
Only what you decide to send. The report for the school or parents follows a minimization principle: plain language, no jargon or diagnosis, and nothing private about the family.
