- Initial assessment — Reason for the visit (what brought the client in), reported lifestyle habits (diet, sleep, activity, stress — only what is said), practitioner's observations (as stated, without a diagnosis), client's goals, and planned program (techniques, frequency).
- Session note — Feelings reported by the client (progress since last session), techniques practiced, reactions observed (as reported, without a medical interpretation), advice given (lifestyle, exercises), and next session.
- Program / follow-up — Review of previous sessions (factual summary), areas of focus chosen, practical recommendations (as stated by the practitioner), and frequency and next deadline.

Does a naturopath account see formats meant for other professions?
No. Every Notae account has a profession chosen at signup. A naturopath account only sees the formats and vocabulary specific to their profession; it has no access to any format or text reserved for another profession (for example a journalist), and vice versa.
