Three formats, your vocabulary
A general practitioner Notae account only sees these three formats — never the formats or vocabulary reserved for another profession, and vice versa.
- Consultation (medical SOAP) — A medical SOAP note: the reported reason for the visit, the clinical exam, the assessment — a diagnosis only if stated by the doctor — and the treatment plan.
- Referral letter — Reason for referral, relevant history (if mentioned), current clinical findings, tests performed/results (if provided), and the question for the colleague or desired follow-up.
- Certificate — Identification of the person examined (reference only, no unrequested diagnosis), factual statement (what was observed or declared, plainly worded), scope of the certificate (what it is issued for — rest, fitness, attendance — as requested), and duration/validity.

Frequently asked questions
Does Notae make a diagnosis in place of the doctor?
No, never. Notae only reports what is said during the consultation; a diagnosis appears in the document only if the doctor explicitly stated it, and no dosage or test result is ever completed or invented.
Does a general practitioner account see formats meant for other professions?
No. Every Notae account has a profession chosen at signup. A general practitioner account only sees the formats and vocabulary specific to their profession; it has no access to any format or text reserved for another profession (for example a psychologist), and vice versa.
Is medical confidentiality respected?
Yes. Documents stay in your encrypted account, and you decide whether to send them to a colleague or a third party; Notae never shares them on its own.
